Draza Martial Arts LLC – Waiver, Release of Liability & Assumption of Risk Agreement  

By participating in any class, program, or activity at Draza Martial Arts LLC, I acknowledge that Brazilian Jiu‑Jitsu, Grappling, Wrestling, Striking, Boxing, MMA, Self‑Defense, Fitness Training, and Kids Classes involve inherent risks, including but not limited to sprains, strains, fractures, dislocations, head/neck injuries, choking, joint locks, submissions, cuts, bruises, impact injuries, serious injury, or death. These risks may result from my own actions, the actions of others, the condition of the facilities or equipment, instruction, drills, sparring, or risks that are known, unknown, or not reasonably foreseeable. I voluntarily choose to participate and fully assume all such risks.

 

To the fullest extent permitted by New Jersey law, I hereby waive, release, and discharge Draza Martial Arts LLC, its owners (including Keita Sullivan and Modrazas), instructors, coaches, trainers, employees, contractors, volunteers, and any facility hosting a Draza Martial Arts class from any and all claims, demands, causes of action, damages, or liability arising from injury, illness, property damage, or death, including those arising from the ordinary negligence of Draza Martial Arts or its staff. I understand this release does not apply to gross negligence or intentional misconduct.

 

I agree to indemnify, defend, and hold harmless Draza Martial Arts LLC from any claims brought by me, my family, my estate, my insurer, or any third party arising out of my participation or my child’s participation. I certify that I (or my child) am in good physical and mental health, have no medical conditions that would impair safe participation, and understand that Draza Martial Arts does not provide medical insurance. I accept full responsibility for all medical expenses resulting from participation.

 

I grant Draza Martial Arts permission to use photos or videos of me or my child for marketing, social media, website content, and promotional materials, unless I revoke this permission in writing. If the participant is a minor, I certify that I am the parent or legal guardian, consent to my child’s participation, assume all risks on their behalf, and agree to all terms of this Agreement.

 

If any part of this Agreement is found unenforceable, the remaining sections shall remain in full force and effect. This Agreement is governed by the laws of the State of New Jersey. By checking the box below or submitting this form, I acknowledge that I have read, understand, and voluntarily agree to all terms of this Waiver, Release of Liability, and Assumption of Risk Agreement.
